YPREP Independent Living Trainer (Full time)
Pay rate: $19.50-$21/hour
Under the direction of the Director of Services (or assigned supervisor), the YPREP Independent Living Trainer (ILT) is responsible for the “hands-on” work that is done to achieve stability and functional improvement in daily living, community involvement and health & wellness. The YPREP ILT works in conjunction with the client and clients support network to provide initial and ongoing assessment in key areas, assist in developing realistic goals, monitor the progress toward goals and maintain proper documentation for activities.
- Use the strengths-perspective and client-focused approach to develop IL goals with each client; work collaboratively with their support network to monitor and adjust goals as needed.
- Collaborate with family/client/child to assess need for skills development in the following areas: income/vocational training/employment, educational planning, housing, parenting, budgeting, skills, increasing independence, physical and mental health needs, social & communication skills and any other identified goals.
- Incorporate the family unit (parent, siblings, child’s father, extended family, identified supports) in service delivery and planning to promote permanency and encourage healthy relationships.
- Utilize Casey Life Skills Assessment and other assessments to provide in-depth analysis of client’s needs as it relates to independent living.
- Collaborate and empower clients to develop educational, vocational, and personal development goals.
- Discuss with youth strengths and areas for improvement related to interpersonal skills (i.e.: communication, emotional management, coping skills, healthy relationships, etc.).
- Provide experiential learning opportunities in the community with every client in order to develop IL skills.
- Collaborate with colleagues and community members during client meetings to meet with needs of the client/family unit.
- Ensure home safety and wellbeing meets and/or exceeds minimum standards to allow for age-appropriate development.
- Provide transportation vouchers, transportation or problem solving related to transportation for appointments.

Work with the client to develop an understanding of the importance of housing, how it is related to maintaining a budget, and the consequences of poor decisions as well as the safety nets available in case the client makes those decisions.
- Assist with finding appropriate housing as well as linkages to appropriate housing services and educational opportunities
*Minimum requirements include Associates Degree with minimally (2) years of experience working with youth and children. Foster Care and/or Residential experiences a plus.

Registered Dietitian (Part time)
Pay rate: $33-$37/hour
Working in collaboration with the Director of Operations, the Registered Dietitian will be responsible for assessing the nutritional status of clients and recommending the appropriate nutrition therapy as it pertains to the clients’ nutritional needs and provide connection between community programs and health related social needs such as food and nutrition insecurity.
- Provide Nutrition Services for WNY including on-site visits, menu development and analysis, and individual and group nutrition counseling and education.
- Conduct Medical Nutrition Therapy with referred clients.
- Interviews clients and/or family for nutritional history, obtains client food preferences, assesses clients, and documents in clients’ medical record.
- Conducts nutritional assessment, develops and implements client specific care plans.
- Provides nutritional guidance and counseling to clients and their families.
- Assists with changes to the client’s menu, including client food preferences, and the development of standardized recipes.
- Prepares, presents, and conducts individual and group education classes, as needed.
*Minimum requirements include being Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR) and licensed by the NY State Dietetics Licensing Board.

Youth Support Specialist
Pay rate: $17.50-$18.50/hour
This position is directly responsible for maintaining a safe and secure environment by monitoring premises, supervising residents, preventing and identifying potential risks, completing appropriate documentation, and ensuring the rights and dignity of each resident is maintained at all times. Additionally, will work on the floor, working in a hands-on capacity. The YSS is also responsible for providing support, positive direction and assistance in accordance with program policies, procedures and personal outcome measures. It is imperative that the YSS provide encouragement, mentoring, positive role modeling, coaching, and guidance to youth/residents at all times. This position requires flexibility, self-management, organization, and a passion to enhance the life of traumatized youth placed in foster care.
*Minimum requirements include a high school diploma/GED and 1 year of paid experience in the human services field. Applicants must have a valid NYS Driver's License and reliable transportation with the ability to transport. Case Coordinator (Full-time)
Pay rate: $21/hour
The Case Coordinator is responsible for the initial and ongoing assesment of all clients in key areas (health, safety and well-being); assisting clients in developing realistic goals; making referrals for services; maintaining positive relationships with community service providers; ensuring permanency and regular family contact; working proactively with the team to ensure productivity; monitoring the progress of clients and for the "hands-on" work that is done to improve and show progression with each clients independent living skills. Utilization of Trauma Informed Care focus is essential in this position.
*Minimum requirements include a bachelor's degree in social work, education, psychology, rehabilitation counseling or related field plus at least 1 year experience in human services, licensure preferred. Applicants must have a valid NYS Driver's License and reliable transportation with the ability to transport.
